Treat suspected exposure as immediate

  1. Prevent further licking. Move the dog away from the spill.

Why the metabolites cause kidney failure

Ethylene glycol itself causes intoxication-like neurological signs. Liver enzymes then convert it into acidic compounds and calcium oxalate, which damage the kidneys and produce severe metabolic acidosis.

The most effective antidotal window is early, before substantial metabolism occurs. Once a dog is producing little or no urine from established kidney failure, prognosis becomes poor.

Diagnostic testing

The clinic evaluates acid-base balance, kidney values, electrolytes, urine concentration and sediment. Calcium oxalate crystals may appear but their absence does not rule out poisoning.

Some hospitals use ethylene-glycol testing or osmolar-gap calculations. A normal early kidney value does not prove safety because renal injury may not yet be measurable.

Important: Activated charcoal binds ethylene glycol poorly and should not create false reassurance. The critical therapy is early professional decontamination and blockade of toxic metabolism.

Antidotes and intensive care

Fomepizole or medically controlled ethanol can inhibit alcohol dehydrogenase and reduce formation of toxic metabolites. Product choice and effectiveness depend on species, timing and availability.

Additional treatment may include intravenous fluids, correction of acidosis, anti-nausea care, electrolyte monitoring and management of kidney failure. Dialysis may be considered where available.

  • Clean spills immediately and block the area
  • Dispose of used fluid through an authorised route
  • Check under parked vehicles for leaks
  • Teach staff that sweet taste does not mean safe
  • Consider less hazardous products while still preventing access
